Agent converts such to PDF and sends it to FTN, this then is treated as a "RFQ." The MS DOC format allows you to play with margins and add your own letter head as well.

2nd May 2012: I will make a confirmation slip for commission payment defined as a promissory note in the next day or so. When such a confirmation slip is issued to sourcing intermediaries 'stepping back', apply this seal.

2nd May 2012: I've made an example of PDF from above MS DOC application. As you can see You add your letterhead, add the commission seal and fill in details then convert to PDF when sending off in similar style as per pre advice in "PN restricted Pdf." Again play around with MS format until you get it working as a master template. It won't be issue often ,but the future is going to be very busy and that other 'assurances" may need to be applied, so it's not just all about commission payments.
